About the Author :
Dr. Bach is a Professor of Physical Medicine and Rehabilitation, Department of Physical Medicine and Rehabilitation, Professor of Neurosciences, Department of Neurosciences, and Medical Director of the Center for Ventilator Management Alternatives and Pulmonary Rehabilitation, University Hospital, Newark, N.J. since 1992. Dr. Bach has been a fellow of the American Academy of Physical Medicine and Rehabilitation, the American College of Chest Physicians, the Association of Academic Physiatrists, the American Paraplegia Society, the New Jersey Association of Electromyography and Electrodiagnosis, l'Association Francaise d'Actions et de Recherches sur les Dystrophies Musculaires, Sigma Xi, Alpha Omega Alpha National Honor Society, the American Spinal Injury Association, and the Institut Duchenne pour la Recherche Dans les Maladies Neuromusculaires. He has 300 peer reviewed scientific publications, 180 book chapters, and 11 books on neuromuscular and pulmonary medicine topics and has lectured on these topics in over 70 countries. Dr. Miguel R. Gonçalves, is a Professor of Pulmonary Medicine of the Faculty of Medicine, University of Porto, Portugal, a Registered Senior Physiotherapist and Clinical Specialist of the Noninvasive Ventilatory Support Unit, Pulmonology Department; Emergency and Intensive Care Medicine Department, and Coordinator of the Portuguese Home Mechanical Ventilation Program (CAI_Vent) that he organized for adults and children. As a respiratory physiotherapist he introduced, and for years was the only clinician in Europe, to employ continuous noninvasive ventilatory support (CNVS) and mechanical insufflation exsufflation (MIE) as a long-term alternative to tracheostomy tubes. He was the first in Europe to extubate and decannulate continuously ventilator dependent patients to CNVS and MIE. He is one of only a handful of clinicians in www.breatheNVS.com centers around-the-world who routinely extubate ventilator unweanable infants with severe neuromuscular disorders to eliminate need for tracheotomies.
