Le Aziende Sanitarie Locali hanno attraversato negli ultimi decenni importanti trasformazioni sia sul piano gestionale che organizzativo e finanziario. Il decentramento e il federalismo amministrativo hanno determinato la nascita di moderne strutture organizzative gestite con nuovi criteri manageriali, sottoposte a serrati controlli sia interni che esterni. Tuttavia la salute, al di la' degli aspetti organizzativi, strutturali e tecnologici resta un campo di scambio tra chi ha un bisogno e chi offre il suo contributo per la risoluzione di questo bisogno. L'importanza del fattore umano resta piu' che mai fondamentale nel comparto salute dove la base dell'iceberg, quello che si nasconde sotto la superficie di nuove strutture, nuovo management, nuove tecnologie, ha forse il peso piu' importante per quanto riguarda il percorso del bisognoso verso la cura e il benessere. Ma al contrario dei fattori hard finora il Sistema Sanitario ha dedicato scarsa attenzione al fattore umano, a quelle che sono i possibili percorsi per motivare ed incentivare il personale verso una completa empatia con i propri fini istituzionali: gli uffici del personale diventati sulla carta "Unita' operative per la gestione e la valorizzazione delle risorse umane" restano ancorati a compiti amministrativi e burocratici che ben poco sanno o fanno per il benessere lavorativo, per la soddisfazione della clientela interna. Dopo un'ampia trattazione teorica sulla definizione dei concetti di clima e cultura organizzativa nel corso degli ultimi decenni ho descritto le principali metodologie di indagine per una ricerca sul clima aziendale. Il suggerimento sempre presente e' stato quello di considerare la ricerca sul clima come "ricerca intervento" come qualcosa non fine a se stessa ma capace di intervenire sulle condizioni di partecipazione emotiva con l'organizzazione sia dal momento della sua realizzazione, segno di attenzione per i lavoratori, ma soprattutto nelle sue conseguenze intese come intervento, appunto, sulle aree critiche emerse dall'indagine. Ho quindi passato in rassegna le ricerche effettuate da alcune Aziende Sanitarie Locali negli ultimi anni, evidenziando come alcuni temi come l'equita', la comunicazione, la valorizzazione siano particolarmente sentiti da tutti i lavoratori. Si tratta di punti critici che Amministrazioni attente dovrebbero avere in bella evidenza nella loro agenda, al fine di programmare interventi che successivamente diventeranno oggetto di ulteriore rilevazione. L'indagine sul clima aziendale infatti deve trasformarsi da evento episodico a intervento sistematico atto a cogliere le zone grigie che possano determinare un pericoloso ammuffimento del personale: e' infatti grande il rischio che tutto il processo di riammodernamento della sanita' italiana vada ad arenarsi per una scarsa partecipazione emotiva e professionale degli operatori.
