Seventy-five percent of survivors reported behavioral and mental health services as their top unmet need when they exited trafficking. Thirty-nine percent still did years later at the time of the survey. Between those two percentages lies the documentary record of what the American anti-trafficking apparatus does not do.

The Survivors is the fifth volume in a six-book investigation of American sex trafficking as an institutional problem. Earlier volumes documented the scale, the platforms, the child welfare pipeline, and the enforcement machinery. This volume examines what happens after identification-the services gap, uncollected mandatory restitution, the criminalization pattern that places trafficked people into the prison system for crimes traffickers compelled, the financial recovery architecture that pauses debt but never extinguishes it, the thin evidence base after twenty-five years of federal funding, and standards of care the federal government began developing in 2024, twenty-four years into the Trafficking Victims Protection Act era.

The Polaris National Survivor Study surveyed 457 survivors and produced the field's most complete post-identification dataset. Sixty-two percent had been cited, arrested, or detained. Seventy-one percent of those carried criminal records. Ninety percent of those records were trafficking-related. The Human Trafficking Institute examined one fiscal year of federal restitution data and found that $257,449 had been collected against $9,166,689 awarded-under 3 percent. The Office for Victims of Crime grantee network expanded to 288 grantees serving 10,505 clients per quarter in FY2024, against an estimated prevalence of 1.1 million. Forty-five states enacted some form of vacatur law by 2024; most eligible survivors cannot reliably access it. The Debt Bondage Repair Act built a federal credit-block pathway effective July 2022; underlying debts remain enforceable.

What works is recorded. Stable housing works. Sustained mental health services work. Peer support works. Vacatur laws work. Credit protections work when enforced. The evidence points one direction. The system moves in that direction. Slowly. Twenty-four years slowly.

The Backpage Remission Program launched July 31, 2025, with more than $200 million in forfeited assets-the largest victim-compensation effort under the TVPA regime. The filing deadline was extended from February 2, 2026 to March 31, 2026 to accommodate continued outreach. Most trafficking convictions do not generate $200 million in recoverable assets. Most produce ordered restitution that is never collected.

This is not advocacy. The volume presents the evidence, the costs, and the barriers. It documents institutional failure through the system's own records-the OVC grantee infographics, CRS reports, the TIP Report's United States profile, the CFPB's supervisory findings, peer-reviewed systematic reviews, and the survivor-authored research base. Every debate is presented with its best available evidence on both sides. No partisan framing. No conspiracy endorsement. No shock value.

The measure of the system is not whether statutes exist, task forces meet, or prosecutions reach a given count in a fiscal year. The measure is whether survivors identified can rebuild their lives after identification.

By that measure, the documentary record speaks.

The Sex Trafficking Files is a six-book investigative nonfiction series examining the institutional response to sex trafficking in the United States. Each volume investigates one dimension: the scale, the commercial platforms, the child welfare pipeline, the enforcement machinery, the survivor services apparatus, and the transnational networks. Each book stands alone. Each is built on the system's own records.

Where the evidence is clear, the books say so. Where it is contested, they present both sides. Where it is absent, they note the gap and move on.
