Critics aruge that there are too many managers in the NHS, with too much money spent on them: at best they are ineffectual; at worst they are positively obstructive. In this Social Market Foundation publication, Nigel Edwards challenges the 'myth of NHS over-management' claim by claim. He argues instead that managers take up a small proportion of the NHS and its budget, that they are valued by staff, and that they are improving services for patients. The debate on NHS reform is dogged by the accusation that money is being wasted on an army of managers and administrators. Commentators decry what they see as a proliferation of 'pen-pushers', who make the lives of doctors and nurses more difficult. The accusation that there are 'more bureaucrats than beds' has become a convenient soundbite, endlessly repeated by those who believe the current reforms are making little impact. Such sloganising might make irresistible headlines. But the claims are not only untrue, they are hugely damaging, to the morale of hardworking NHS staff, and to the quality of the health debate. We need a more honest and sophisticated analysis of the real challenges facing the health service, and a proper debate about the role that management should play in delivering better patient care. This essay is an attempt to do just that. It starts by examining the key charges against NHS management: that there are too many managers, that new money has been wasted on bureaucracy, and that NHS managers make little difference to patient care. It finds that these claims are misleading. It then considers why the belief that the NHS is full of managers has arisen. It concludes that there is an argument that the NHS has in fact been significantly under managed in many areas and that a number of the improvements that are needed in the NHS require more, not less, management.
