Abstract: Abstract Abstract Abstract of the thesis entitled CONDYLAR ADAPTATION TO ACTIVE MANDIBULAR FORWARD POSITIONING IN NON-GROWING RATS Submitted by Xiong Hui, BDS, MDS For the degree of Doctor of Philosophy at The University of Hong Kong in July 2004 Knowledge concerning growth adaptation in adults is limited. Therefore, this study was designed to assess condylar adaptation to active mandibular forward positioning in adult rats by quantifying changes in the number of mesenchymal cells and their rate of proliferation; changes in expression of SOX9, VEGF, type II and type X collagens; changes in the level of new bone formation; changes in mandibular and condylar morphology; and the microstructural properties of cancellous bone in the condyle. Seventy-eight 120-day-old female Sprague-Dawley rats were used in this study. The rats were randomly allotted to six groups. Each group consisted of nine rats with bite-jumping devices and four untreated animals. The animals in each group were sacrificed on days 3, 7, 14, 21, 30 and 60 respectively. Half of the mandibles were taken for evaluation of gross morphological changes and properties of microstructure of cancellous bone in the mandibular condyle. The others were subjected to histological assessment. Cell kinetic studies for expression of PCNA were used to identify the number of proliferating mesenchymal cells and the rate of proliferation. Col2a1 was identified by in situ hybridization. Immunostaining was used for the detection of SOX9 and type II and type X collagen, and Alcian blue-PAS was used to I Abstract observe the extracellular cellular matrix and new bone formation. Endochondral ossification was reactivated in the adult posterior condyle. The proliferative layer became thicker on experimental day 21. The thickness of the chondroblast layer and chondrocyte layer increased significantly between experimental day 14 and day 30. There was also a significant increase in the number of replicating mesenchymal cells and their proliferation rate. The expression of SOX9 was enhanced and Col2a1 gene transcript was activated. The highest level of expression of Type II and Type X collagen occurred on day 21, and the levels of increase were 248% and 540% respectively. The expression of VEGF was up-regulated. The highest level of new bone formation was measured on day 30, and the level of increase of new bone formation was 319%. As identified by micro-CT, the new bone formed in the condyle was characterized by thinner trabecular thickness, a higher trabecular number, and increased trabecular space. Measurements of the mandible revealed an increase in condylar length and change of direction of growth. In conclusion, forward mandibular positioning in adult rats induces mechanical strain on the condyle, which promotes the proliferation of mesenchymal cells. These mesenchymal cells are then committed to enter the chondrogenic route, producing condylar growth in adult rats.
