Abstract: Abstract of thesis entitled "Tumour infiltrating lymphocytes (TILs) - a prognostic factor for gastric adenocarcinoma" Submitted by CHOI Ka Man for the degree of Master of Philosophy at The University of Hong Kong in July 2005 Gastric cancer is one of the most common malignancies worldwide. Despite a universal decrease in the incidence rate and mortality rate of this disease, its prognosis remains poor, with an overall 5-year survival rate of lower than 25%. The histological stage of gastric cancer remains the most important determinant of prognosis. It has been shown that tumour infiltrating lymphocytes (TILs) are effective in eliminating tumour cells. The prognostic value of TILs has been studied in various cancers, but remains to be fully elucidated in gastric adenocarcinoma. Eighty paraffin-embedded specimens of gastric adenocarcinoma were examined in this study. The distribution of TILs was assessed by immunohistochemistry. It was found that CD3+ T cells, CD4+ T cells, CD8+ T cells, CD20+ B cells, CD57+ natural killer cells, and CD45RO+ memory/activated T cells aggregated within tumour areas. A higher level of CD3+ TILs was associated with the earlier stages of tumour (P = 0.007). There was a significantly better survival in patients with a high level of CD3+ or CD45RO+ TILs than those with a low level. Apart from the T staging ( P = 0.017; hazard ratio = 2.285; 95% CI = 1.162-4.496), N staging ( P = 0.043; hazard ratio = 2.419; 95% CI = 1.030- 5.683), and M staging ( P = 0.001; hazard ratio = 10.272; 95% CI = 2.554-41.311), the degree of CD45RO+ memory/activated T cells infiltration was also found to be an independent prognostic factor ( P = 0.019; hazard ratio = 0.261; 95% CI = 0.085- 0.804). Molecular analyses were performed on eleven tumour specimens by real- time quantitative polymerase chain reaction to investigate their cytotoxic functions. The mRNA level of the CD3 epsilon chain and IFN-gamma, a key cytokine for antitumour immune responses, were found to be abundant in rich CD3+ T cell specimens ( P = 0.018 and P = 0.016, respectively). In conclusion, this study shows that a high level of CD3+ TILs correlates with an earlier histological tumour stage. It also demonstrates that an increased count of CD3+ or CD45RO+ TILs was significantly associated with improved patient survival. The development of CD3+ and CD45RO+ as prognostic markers for gastric adenocarcinoma should provide important insights into the nature of this disease, and may eventually lead to the development of effective treatment modalities, such as immunotherapy.
