It would appear that such disturbance is very largely motor, possibly somewhat secretory. II. Symptomatology of the System Diseases. By the term system diseases we understand, as has been said above, such processes as involve symmetrically lying tracts or neurons which are functionally and anatomically connected, or (in the beginning) parts of neurons. Eventually the disease does not confine itself to a portion of a neuron (cell or fibre) but the entire neuron becomes the seat of disease. The localization of the primary seat of the disease is frequently of great importance in pointing the clinical course. Concerning the direction in which this process extends and as to its point of origin there is yet a great deal of obscurity. 1. System Diseases of the Cortico-Muscular Tract: the Motorial Pathway. a. Of the central neuron: purely spastic spinal paralysis. Cases of this kind are of rare occurrence. Anatomically there is a slow, progressive degeneration of the pyramidal tract in its entire course, the starting-point being the pyramidal cells of the cortex (?). Clinically there is a slowly developing spastic paralysis of the extremities, with lively reflexes, without muscle atrophy and without disturbances of sensibility or of the bladder. b. Of the peripheral neuron: there results a flaccid paralysis of the muscles with degenerative muscle atrophy. The reflexes are lost. The primary seat may be either in the neuron cell or in the neuron fibre. a. Primary diseases of the cell.
