About the Book

Hazard identification and analysis constitute part of the process which assures the safety of a system. It is extensively applied to plant and operation in a number of industries, but there has been limited practical application to the development and operation of safety related computer systems. This report provides guidance on the techniques that should be applied for performing hazard identification and analysis activities during the design, development, operation, maintenance and decommissioning of a safety related sytem incorporating programmable electronic systems (PES). It is based on practical experience and theoretical approaches. Both the functional and physical hazards associated with the deployment of PES are addressed. Particular attention is paid to operating and maintenance aspects of a design. Practical advice is given on the advantages and disadvantages of the application of techniques at different stages of the safety lifecycle. Typical problems and the ways in which they may be overcome are described. The phases of the system development at which these hazard identification and analysis activities should be performed are described in relation to the IEC 61508 saf Appendices provide: an overview of other activities within the safety lifecyle and how they relate to the hazard analysis activities, an overview of a selection of techniques which can be used for the conduct of hazard analysis activities, and an example of the application of some of these techniques to the development of a traffic light controller. These guidelines are aimed at practitioners who have experience in hazard identification and analysis techniques but who are not familiar with their application to PES. For those with less experience further assistance is provided in the appendices.
